I've managed to connect

However since I'm using a Sprint SIM card it goes to Searching, then goes to No Service, after a minute it picks up a signal.
I don't know if I have the right number as I activated this phone a while ago on one of those free Page Plus activation websites (Remote Squad I think) but changing the PhoneRegistrationGatewayAdress to the Page Plus one (it starts with a +4xxxxxxxxxx) it gave me some number in my Phone app

It could be working at this point for talk and text but I can't figure out how to change the MSID (IMSI), its stuck on 0000004996. Tried using the "KeypadProvisioningNumber" string with ##626 and ##626# (just to make sure) but it doesn't work? Maybe they changed it?
Anyways as far as I can tell that's the only roadblock I have towards having a fully talk and text capable iPhone 5 as I can connect to the Page Plus network

I don't know if 3G will work, but I think it should as long as the MSID is set since the NAI and everything are the same (with a Verizon SIM card). Right now I don't expect it to work on the Sprint SIM card as the SIM card sets the NAI to [ESN]@hcm.sprint.com but with a Verizon SIM card it should work just fine.
This is for the Verizon iPhone 5 (Clean ESN) with a deactivated Sprint SIM card, I'm going to test out a Verizon SIM card soon although I don't expect it to work since it's still active on my account.
The LTE CDMA SIM cards really help since you can use different Carrier Bundles.
